The thyroid gland produces two primary hormones — T3 and T4 — that regulate metabolic rate across virtually every cell in the body. When thyroid output is insufficient, resting metabolic rate falls, fat oxidation slows, gut motility decreases, and body temperature drops. People with untreated or undertreated hypothyroidism can eat very little and still gain weight — not because of weak willpower, but because their metabolic machinery is operating below capacity.

How hypothyroidism impairs fat metabolism

T3 (triiodothyronine) is the metabolically active thyroid hormone. It upregulates uncoupling proteins in mitochondria, increasing thermogenesis; it enhances lipolysis (fat breakdown) in adipose tissue; and it increases the number and sensitivity of beta-adrenergic receptors that respond to fat-mobilizing signals. Low T3 reverses all of these effects. Resting metabolic rate can fall 20–30% in overt hypothyroidism, with proportional reductions in fat oxidation capacity.

Hypothyroidism also slows gut motility, which affects nutrient absorption patterns and microbiome composition. It increases LDL cholesterol (because thyroid hormone normally upregulates LDL receptors in the liver) and promotes fluid retention through effects on aldosterone and the renal tubules. The combined effect is a metabolic environment where fat loss is significantly harder regardless of caloric intake.

Symptoms that suggest thyroid involvement

Common indicators of hypothyroidism include: persistent fatigue despite adequate sleep, unexplained weight gain or inability to lose weight despite dietary restriction, cold intolerance, constipation, dry skin and hair loss, depression or cognitive slowing, and elevated LDL on blood work with no dietary explanation. TSH (thyroid-stimulating hormone) is the standard screening test; elevated TSH indicates the pituitary is working harder to stimulate an underperforming thyroid. A physician assessment is necessary for diagnosis and treatment decisions.

The role of dietary support

Dietary support cannot replace thyroid hormone replacement therapy when it is medically indicated. However, dietary factors do influence thyroid function. Iodine deficiency impairs T3/T4 synthesis. Selenium deficiency impairs the conversion of T4 to the active T3. Chronic caloric restriction suppresses T3 production as an adaptive response — which is why aggressive restriction often backfires metabolically. Foods high in goitrogens (raw cruciferous vegetables in large quantities) can reduce iodine uptake in thyroid tissue, though cooking largely neutralizes this effect.
