Standard lipid panels measure LDL cholesterol concentration — but what matters for cardiovascular risk is LDL particle number and size, not cholesterol content per particle. Two people with identical LDL-C can have very different ApoB levels and very different cardiovascular risk profiles. This gap is largest in people with insulin resistance, where the most dangerous lipid pattern (small dense LDL, elevated triglycerides, low HDL) coexists with apparently normal LDL-C.

ApoB: counting particles, not cholesterol content

Each LDL particle contains exactly one ApoB molecule. ApoB is therefore a direct count of atherogenic particles — the particles that penetrate arterial walls and deposit cholesterol in plaques. Large-scale studies including MESA and Copenhagen Heart Study consistently show ApoB outperforms LDL-C as a cardiovascular predictor, particularly at triglycerides above 150 mg/dL.

How insulin resistance creates the most dangerous pattern

Insulin resistance elevates hepatic VLDL production, raising triglycerides and shifting LDL toward smaller, denser particles. Small dense LDL (sdLDL) oxidizes more easily, crosses endothelium more readily, and circulates longer (reduced LDL receptor affinity) — all increasing atherogenic risk. The triglyceride:HDL ratio above 2.0 (mg/dL) is a validated surrogate for this sdLDL-dominant pattern in people with insulin resistance.

Reducing dietary glycemic load consistently improves the atherogenic lipid pattern: triglycerides fall, HDL rises, and LDL particles shift toward larger, less atherogenic forms — independent of total LDL-C changes.
