Standard health checkups measure fasting blood glucose — but glucose rises late in the progression of insulin resistance, often not crossing diagnostic thresholds until 50% of beta cell function is already lost. Fasting insulin and HOMA-IR (homeostatic model assessment of insulin resistance) detect the underlying insulin resistance far earlier, providing a meaningful prevention window that glucose-only screening misses.
Fasting insulin: what it measures
Fasting insulin measures how much insulin the pancreas must produce to maintain normal fasting glucose. In insulin-sensitive individuals, fasting insulin is typically 3–8 μIU/mL. As insulin resistance develops, fasting insulin rises to maintain glucose control — often reaching 15–25 μIU/mL before fasting glucose shows any abnormality. This elevated fasting insulin is the metabolic system working overtime to compensate for reduced cellular responsiveness; it is both a marker of insulin resistance and one of its drivers (chronic hyperinsulinemia further reduces insulin receptor sensitivity).
HOMA-IR: the combined index
HOMA-IR = (Fasting insulin [μIU/mL] × Fasting glucose [mmol/L]) / 22.5. Values below 1.0 indicate high insulin sensitivity; 1.0–2.0 is normal; above 2.0 suggests insulin resistance; above 3.0 indicates significant insulin resistance. HOMA-IR is the most widely validated clinical measure of insulin resistance short of the gold standard euglycemic-hyperinsulinemic clamp, and it correlates with metabolic syndrome severity, cardiovascular risk, and T2D progression across populations.
What these numbers mean for dietary decisions
Elevated fasting insulin and HOMA-IR in the absence of elevated glucose represents the most actionable window for dietary intervention: metabolic dysfunction is present and progressing, but beta cell function is preserved and the damage is reversible through sustained dietary restructuring.
