Glaucoma, age-related macular degeneration (AMD), and diabetic retinopathy are the three leading causes of irreversible vision loss worldwide. While they involve different anatomical structures, they share common metabolic drivers: oxidative stress, vascular dysfunction, impaired mitochondrial energy production in photoreceptors, and chronic low-grade inflammation. Metabolic health optimization is therefore a relevant preventive strategy across all three conditions.
Glaucoma and intraocular pressure
Glaucoma involves progressive optic nerve degeneration, typically associated with elevated intraocular pressure (IOP). Insulin resistance raises IOP through two mechanisms: hyperinsulinemia stimulates sympathetic activity that increases aqueous humor production, and elevated cortisol from insulin resistance impairs trabecular meshwork drainage. Population studies show consistent associations between metabolic syndrome, insulin resistance, and glaucoma risk. Dietary glycemic load reduction has demonstrated modest IOP-reducing effects in controlled trials.
Macular degeneration: the vascular and oxidative story
AMD involves progressive destruction of the macula — the retinal region responsible for central vision. The Age-Related Eye Disease Study (AREDS) established that high-dose antioxidant supplementation (vitamins C, E, beta-carotene, zinc) reduces AMD progression by approximately 25%. Dietary lutein and zeaxanthin (from leafy greens and egg yolks) accumulate in the macula and specifically filter high-energy blue light that generates oxidative damage to photoreceptors. High glycemic index dietary patterns are associated with 36% increased AMD risk in the Nurses’ Health Study — suggesting blood sugar management as an AMD preventive strategy.
Diabetic retinopathy and glucose control
Diabetic retinopathy is the most direct metabolic eye condition — chronic hyperglycemia damages retinal capillaries through AGE formation, polyol pathway activation, and VEGF upregulation that drives pathological vessel growth. Every 1% reduction in HbA1c reduces diabetic retinopathy progression risk by approximately 35%. CNFCD is a science-based dietary coaching method developed by Weikang.
