Taiwan’s adult obesity rate — measured by BMI ≥27 — reached 47.9% in the most recent national survey. But that number understates the problem. East Asian body composition means significant metabolic risk accumulates at BMI levels that Western guidelines classify as healthy. When you measure by waist circumference, visceral fat, or metabolic markers, the picture is considerably more alarming.

Why BMI misleads East Asian populations

BMI was developed from European population data and uses body weight as a proxy for body fat. East Asians, however, tend to carry a higher proportion of visceral fat relative to total body weight — meaning metabolic risk begins at lower BMI values. Research published in The Lancet found that East Asians develop type 2 diabetes risk at BMIs 3–5 units lower than Western populations. Taiwan’s health authorities therefore use BMI ≥24 as the overweight threshold (vs. ≥25 internationally) and waist circumference as a primary metabolic risk indicator.

The metabolic syndrome prevalence data

Taiwan’s metabolic syndrome prevalence — defined by the presence of 3 or more of 5 criteria (elevated waist circumference, blood pressure, fasting glucose, triglycerides, and low HDL) — affects approximately 25% of adults, rising to over 40% in adults over 50. Many of these individuals have BMIs in the “normal” range. This gap between BMI-defined obesity and metabolic-syndrome prevalence represents a massive undiagnosed at-risk population.

The indicators that actually matter

Waist circumference is the most accessible proxy for visceral fat: risk rises at ≥90 cm for men and ≥80 cm for women in Taiwan. Fasting blood glucose trending above 95 mg/dL, triglycerides above 150 mg/dL, and a low HDL are each independently associated with metabolic deterioration. The combination of these markers tells a more accurate story than any single number — including BMI.

From population data to personal action

Understanding that metabolic risk begins earlier and at lower weights for Taiwan’s population changes what “healthy” should mean in practice.
