Insulin resistance elevates blood pressure through several parallel mechanisms. First, it activates the sympathetic nervous system: insulin normally counteracts sympathetic activity, so insulin resistance allows sympathetic tone to remain elevated, constricting blood vessels and raising heart rate. Second, insulin resistance impairs the kidney’s ability to excrete sodium, promoting fluid retention and increasing blood volume. Third, chronically elevated insulin stimulates endothelin-1 (a vasoconstrictor) and suppresses nitric oxide (a vasodilator), shifting the vascular balance toward constriction. Research consistently shows that insulin sensitivity improvement — through dietary restructuring — reduces blood pressure independently of weight loss.
The fructose-hypertension connection
Fructose metabolism in the liver generates uric acid, which constricts renal afferent arterioles — directly raising intraglomerular pressure and systemic blood pressure. A 2010 clinical trial (Jalal DI et al.) showed that reducing added fructose intake by 74g/day over 6 weeks significantly reduced both systolic and diastolic blood pressure. Taiwan’s high bubble tea and sweetened beverage consumption represents a significant hidden fructose load contributing to hypertension prevalence.
