Alcohol is metabolized almost entirely in the liver, where its oxidation takes priority over all other metabolic processes. This prioritization displaces fat oxidation and diverts metabolic capacity to alcohol clearance — with downstream consequences for liver fat, gut health, hormone balance, and sleep quality.

Liver: fat accumulation during alcohol metabolism

Alcohol is oxidized to acetaldehyde then to acetate, generating excess NADH that shifts the liver toward fat synthesis and away from fat oxidation. Even moderate regular drinking measurably increases hepatic fat content, worsening over time in people with pre-existing fatty liver.

Gut: microbiome disruption and increased permeability

Alcohol disrupts gut microbiome composition directly — it has broad antimicrobial effects and reduces tight junction protein expression (particularly occludin and claudin-3), increasing intestinal permeability. LPS from gut bacteria enters systemic circulation, activating TLR4-driven liver inflammation that contributes to alcohol-related liver disease even without overt cirrhosis.

Hormones and sleep: the cortisol rebound

Alcohol initially suppresses cortisol — producing relaxation — but triggers a rebound elevation 4–6 hours later that disrupts sleep architecture, suppresses slow-wave sleep, and drives post-alcohol hunger. It also suppresses testosterone in men, disrupts estrogen metabolism in women, and blunts growth hormone pulsatility during sleep.
