The gut barrier degrades when butyrate production is insufficient (due to low dietary fiber), when the microbiome loses diversity, or when chronic stress, alcohol, or NSAIDs disrupt tight junction proteins like claudin and occludin. Modern diets high in ultra-processed foods and low in fermentable fiber are a primary driver. Once the barrier is compromised, the resulting endotoxemia — chronic low-level LPS in the bloodstream — activates the immune system continuously.

1. Obesity and metabolic syndrome
LPS activates Toll-like receptor 4 (TLR4), triggering inflammatory signaling that directly impairs insulin sensitivity. Research shows that obese individuals have significantly higher circulating LPS than lean individuals — and that reducing LPS improves metabolic markers.

2. Type 2 diabetes
The chronic inflammation from endotoxemia worsens insulin resistance at the cellular level, accelerating the progression from prediabetes to full type 2 diabetes. Animal studies have demonstrated that transplanting a high-LPS microbiome induces insulin resistance even on a normal diet.

3. Non-alcoholic fatty liver disease (NAFLD)
