Autophagy — the process by which cells break down and recycle damaged organelles, misfolded proteins, and cellular debris — is a primary cellular anti-aging mechanism. It is suppressed by mTOR (activated by constant eating, particularly protein and carbohydrate) and activated by AMPK (activated by fasting, caloric restriction, and exercise). The CALERIE trial — the most rigorous human caloric restriction study — showed that modest 12% caloric restriction sustained for 2 years improved cardiometabolic risk markers, reduced thymic aging (an immune system aging marker), and produced favorable changes in aging biomarkers. This provides the first direct human evidence for benefits beyond metabolic improvements from sustained caloric restriction — though the calorie reduction required (12%) is modest rather than extreme.

Consistent evidence from epidemiology and intervention studies points toward: adequate regular fasting intervals between meals (supporting autophagy), high dietary polyphenol intake (SIRT1 activation, anti-inflammatory), sufficient protein for muscle preservation (preventing sarcopenia), and metabolically healthy body composition (reducing senescent cell burden from visceral fat).
