The evidence base has grown substantially but remains more limited than popular claims suggest. A 2022 randomized controlled trial (Yoshino M et al., Science) found that NMN supplementation (300mg/day for 10 weeks) in postmenopausal women with prediabetes improved muscle insulin sensitivity and increased muscle NAD+ metabolome — a meaningful metabolic effect. A 2023 trial in healthy older adults found that NR supplementation increased blood NAD+ metabolites but produced no significant improvements in physical performance, cognitive function, or metabolic markers compared to placebo. The picture that emerges: NAD+ precursors reliably increase circulating NAD+ metabolites, but converting that increase into measurable clinical benefit — particularly in healthy individuals — is inconsistent across trials.
Where the evidence is stronger
The strongest human evidence supports NAD+ precursor use in specific contexts of metabolic dysfunction: insulin resistance, obesity, and potentially acute muscle recovery. In metabolically compromised individuals, restoring NAD+ may support mitochondrial function that has deteriorated below a functional threshold. In healthy individuals, the incremental benefit above baseline NAD+ levels is less clear.
The dietary path to NAD+ support
NAD+ precursors are found in whole foods: niacin (vitamin B3) from meat, fish, and legumes; tryptophan from protein-containing foods (converted to NAD+ through the kynurenine pathway).
