Visceral fat is metabolically active adipose tissue with a unique anatomical position: free fatty acids released by visceral fat drain directly into the portal vein and reach the liver first, promoting hepatic VLDL production, triglyceride synthesis, and inflammation. Visceral fat also secretes adipokines — including TNF-α, IL-6, resistin, and angiotensinogen — that promote vascular inflammation, endothelial dysfunction, hypertension, and insulin resistance. A 2022 meta-analysis in the Journal of the American Heart Association found that visceral fat volume was associated with a 52% higher cardiovascular event risk independently of BMI — confirming that visceral fat contributes to cardiovascular risk beyond what weight alone captures.
Waist circumference as a practical proxy
